Easy to Follow Instructions
1. Fill in your Name, Shipping information, and Phone #.
2. Next fill in the item #, Description, Qty, and price of the item(s) you want.
3. Then add up the total of the items and put that number in the sub-total space.
4. After that check how much it will cost for shiping. For non poultry items shipping is $5.00.
5. Add togeter the sub-total to the shipping and put this number into the grand total space.
6. Finally print this form out and put your payment of cash, check, or money order into an evelope and ship it off to us. When we receive your order we will process and ship it off as soon as we can.
if you pick up the chicks yourself you do not have to fill in the shipping space.
